other بررسی دامنه حرکتی زانو پس از ترمیم بافت نرم آن در دو وضعیت فلکسیون و اکستانسیون در حین عمل تعویض مفصل زانو: یک کارآزمایی بالینی تصادفی ارتوپدی و تروما Orthopedics and Traumatology مقاله اصیل Original Article <div><span style="font-family:yekanYW;"><strong><em><span style="font-size:10.0pt;">زمینه و هدف:</span></em></strong> <span style="font-size:10.0pt;">پوزیشن بیمار در هنگام ترمیم پس از جراحی آرتروپلاستی می‌تواند به‌عنوان یک عامل در کاهش عوارض و بهبود بیماران باشد. این مطالعه با هدف مقایسه دامنه ‌حرکتی زانو پس از ترمیم بافت نرم آن در دو وضعیت فلکشن و اکستنشن در حین عمل تعویض مفصل زانو انجام‌شد.</span><span style="font-size:8.0pt;"></span><br> <strong><em><span style="font-size:10.0pt;">روش بررسی:</span></em></strong> <span style="font-size:10.0pt;">در این مطالعه کارآزمایی ‌بالینی دوسوکور 88 بیمار کاندید عمل تعویض کامل مفصل ‌زانو در بیمارستان‌های الزهرا و کاشانی اصفهان از تیر 1390 تا شهریور 1391 انتخاب و به‌صورت تصادفی به‌روش بلوک‌های چهارتایی (نسبت یک به یک) در دو گروه قرار گرفتند. در گروه اول در هنگام ترمیم بافت نرم زانو، زانو در وضعیت فلکسیون و در گروه دوم در وضعیت اکستانسیون قرار داده‌‌شده و سپس ترمیم صورت می‌گرفت. اطلاعاتی مانند سن، جنس، شاخص ‌توده</span> <span style="font-size:10.0pt;">‌‌بدنی و سمت مبتلا در چک ‌لیست ثبت گردید. دامنه‌ حرکتی زانو در پایان هفته اول، دوم، چهارم، هشتم و دوازدهم توسط پزشک اندازه‌گیری و ثبت شد. </span><span style="color:black;"><span style="font-size:10.0pt;">مقایسه الگوی تغییرات در میانگین دامنه‌ حرکتی زانو در طول زمان در دو گروه به‌ کمک مدل‌های خطی ‌آمیخته و مدل‌های حاشیه‌ای توسط نرم‌افزار </span></span><span dir="LTR"><span style="color:black;"><span style="font-size:8.0pt;">STATA</span></span></span><span style="color:black;"><span style="font-size:10.0pt;"> انجام‌ شد.</span></span><span style="font-size:8.0pt;"></span><br> <strong><em><span style="font-size:10.0pt;">یافته‌ها:</span></em></strong> <span style="font-size:10.0pt;">اختلاف آماری معناداری بین دو گروه فلکسیون و اکتانسیون در دامنه‌ حرکتی زانو وجود نداشت. در هر دو گروه با گذشت زمان میانگین دامنه ‌حرکتی زانو روند افزایشی داشت (001/0</span><span style="font-size:8.0pt;">></span><span dir="LTR"><span style="font-size:8.0pt;">P</span></span><span style="font-size:10.0pt;">). میانگین نمرات عملکرد زانو پس از پایان هفته دوازدهم در گروه فلکسیون و اکستانسیون برابر با 19/23</span><span style="font-size:8.0pt;">&plusmn;</span><span style="font-size:10.0pt;">04/141 و 34/22</span><span style="font-size:8.0pt;">&plusmn;</span><span style="font-size:10.0pt;">09/143 بود و این اختلاف معنا‌دار نبود.</span><span style="font-size:10.0pt;"></span><br> <strong><em><span style="font-size:10.0pt;">نتیجه‌گیری: </span></em></strong><span style="font-size:10.0pt;">ترمیم بافت نرم در یک پوزیشن خاص چندان تاثیرگذار نمی‌باشد که بخواهیم با صرف هزینه‌های مختلف برای آموزش جراحان، آن‌ را نهادینه کنیم.</span></span></div> <div><strong><em>Background:</em></strong> The patient&#39;s position during the recovery time after arthroplasty surgery can be a factor in reducing complications and improving patients&rsquo; condition. Therefore, this study was performed to compare the knee range of motion after soft tissue repair in both flexion and extension positions of total knee arthroplasty.<br> <strong><em>Methods:</em></strong> This study is a double-blind clinical trial. The study population was all candidates for total knee arthroplasty surgery in Al-Zahra and Kashani hospitals in Isfahan from July 2011 to September 2012. The sample size was selected using the statistical formula of 88 patients and randomly divided into two groups of flexion and extension. In the first group during the knee soft tissue repair, the knee was in flexion and in the second group it was in extension and then it was repaired. Information such as age, sex, body mass index and surgical position were recorded in the checklist. The knee range of motion was measured at the end of the first, second, fourth, eighth and twelfth weeks and recorded in a checklist. In order to compare the pattern of changes in mean knee range of motion over time in the two groups, mixed linear models and conventional marginal models to generalized estimator equations were used by STATA software.</div> <table align="center" cellpadding="0" cellspacing="0" hspace="0" vspace="0"> <tbody> <tr> <td align="left"> <div><strong><em>Results:</em></strong> The results showed that there was no significant difference between the flexion and extension groups in knee range of motion. In both groups, the mean knee range of motion increased with time (P<0.001).
