other بررسی فراوانی بیان نشانگر HER2 و عوامل همراه آن در کارسینوم‌های یوروتلیال Examining the frequency of expression of HER2 marker and its associated factors in urothelial carcinomas مقاله اصیل Original Article زمینه و هدف: سرطان یوروتلیال، نهمین سرطان شایع در جهان است و به‌عنوان هفتمین بدخیمی شایع در مردان شناخته شده است. هدف از این مطالعه، بررسی فراوانی بیان نشانگر HER2 و عوامل همراه آن در بیماران مبتلا به کارسینوم یوروتلیال است. روش بررسی: در این مطالعه مقطعی، نمونه مورد بررسی بیماران مبتلا به کارسینوم یوروتلیال (Urothelial carcinoma) جراحی شده به روش‌های (TURBT) Transurethral resection of bladder tumor و سیستکتومی رادیکال (Radical cystectomy) در بیمارستان مدرس از فروردین 1399 لغایت اسفند 1402 بودند. بلوک‌های پارافینی بیماران استخراج و کنترل شده سپس از نظر بیان نشانگر (HER2)  Human epidermal growth factor Receptor 2به‌روش ایمونوهیستوشیمی Immunohistochemistry (IHC) بررسی شدند. یافته‌ها: در مجموع 84 نفر در این مطالعه شرکت کردند که 74 نفر (1//8%) مرد و 10 نفر (9/11%) زن بودند. اکثر بیماران مورد مطالعه (0/94%) دارای کارسینومای یوروتلیال Conventional بوده، از نظر گرید بیماری 5/84% بیماران دارای تومور با گرید بالا و از نظر مرحله بیماری PT1 Tumor invades subepithelial connective tissue (lamina propria) دارای بیشترین و PT4a (Tumor invades prostate, uterus, or vagina) دارای کمترین فراوانی بوده‌اند. به‌ترتیب 44 و 56 بیمار جراحی برداشت تومور مثانه از مسیر مجرای ادراری (Trans urethral resection of bladder tumour, TURT) و سیستکتومی رادیکال (Radical cystectomy) انجام داده‌اند. در 19% از کل نمونه‌های بررسی شده بیان مارکر HER2 مثبت بود. نتیجه‌گیری: بیان HER2 ممکن است اطلاعات تعیین‌کننده پیش‌آگهی بیشتری را برای بیماران مبتلا به کارسینوم یوروتلیال فراهم کند. اجماع الگوریتم امتیازدهی ایمونوهیستوشیمی HER2 برای اطمینان از اینکه نشانگر زیستی خط اول خوبی برای تعیین انتخاب بیماران برای مطالعات مولکولی و انتخاب آنها برای درمان هدفمند، بسیار مهم است.   <div> Background: Urothelial cancer is the ninth most common cancer in the world and is known as the seventh most common malignancy in men. Clinically, non-muscular urothelial carcinomas (N-mUC) account for 70-75% of bladder cancers. Various factors have been investigated in Relationship with the diagnosis of urothelial cancers, one of the most useful of them is Human epidermal growth factor receptor-2 (HER2) marker, whose relationship with breast, stomach and esophagus cancers has been evaluated in many studies. The aim of this study was to investigate the frequency of expression of HER2 marker and its accompanying factors in urothelial carcinomas. Methods: All patients with urothelial carcinoma with history of Transurethral Resection of Bladder Tumor (TURBT) and radical cystectomy surgery in Modares Hospital in Tehran from April 2020 to March 2023 enrolled in this cross-sectional study. Paraffin blocks of patients were extracted and checked by a pathologist to ensure correct diagnosis. Then by HER2 marker with IHC method, were tested. Results: A total of 84 people participated in this study, of them 74(88.1%) were male and 10(11.9%) were male. Most of the studied patients (94.0%) had urothelial carcinoma without squamous differentiation. In terms of tumor grade, 84.5% of patients have high tumor, and in terms of tumor stage, PT1 has the highest frequency and PT4a has the lowest frequency. 44 & 56 patients have performed TURT and Radical Cystectomy, respectively. HER2 marker expression was positive in 19% of the examined samples. Conclusion:HER2 expression may provide additional prognostic information for patients with urothelial carcinoma.
