other بررسی MRI زانو از نظر ارتباط هم‌راستایی کشککی‎-رانی با نقص‌غضروفی مفصل زانو A knee MRI study to investigate the correlation between patellofemoral alignment and knee joint cartilage defects مقاله اصیل Original Article زمینه و هدف: هدف از مطالعه حاضر، بررسی ارتباط هم‌راستایی مفصل‌زانو و نقص‌غضروف مفصل کشککی-رانی توسط تصاویر MRI می‌باشد. روش بررسی: در این مطالعه‌ی مقطعی، به‌صورت گذشته‌نگر 530 تصویر MRI از زانوی افراد بالای 30 سال سن که به بیمارستان‌های گلستان و امام‌خمینی در شهر اهواز‌، خوزستان، ایران، در بازه زمانی فروردین 1402 الی فروردین 1403 به‌دلیل درد زانوی غیرضربه‌ای مراجعه نموده‌بودند، مورد بررسی قرارگرفت. پارامترهای زاویه شیاری استخوان ران، عمق شیاری استخوان ران، جابجایی جانبی کشکک، زاویه جانبی پاتلوفمورال، فاصله شیار توبرکل درشت‌نی-تروکلئر، زاویه‌ی شیب کشکک و ارتفاع کشکک مورد بررسی قرار گرفت. علاوه‌براین، شاخص Caton-Deschamps و همچنین درجه نقص‌غضروف کانونی در مفصل زانو مورد بررسی قرار‌گرفت. یافته‌ها: از لحاظ شاخص توده‌ی بدنی، 7/15% از بیماران مورد مطالعه دچار کمبود وزن، 3/42% وزن مناسب، 1/31% اضافه وزن، 4/10% دچار چاقی، 6/0% فوق‌العاده چاق بودند. بین شاخص توده‌ی بدنی و نقص‌غضروف مفصلی-جانبی کشککی، درشت‌نی و رانی ارتباط معناداری وجود داشته است. همچنین، بین سن و نقص‌غضروف مفصلی-جانبی درشت‌نی و رانی ارتباط معناداری وجود دارد. بین نقص‌غضروف مفصلی-داخلی و مفصلی-جانبی کشکک با دررفتگی جانبی کشکک، زاویه جانبی کشککی-رانی، فاصله برجستگی درشت‌نی-شیار قرقره‌ای و زاویه شیب کشکک ارتباط معناداری وجود داشت. علاوه‌براین، بین نقص‌غضروف مفصلی- داخلی کشکک با زاویه شیاری زانو نیز ارتباط معناداری وجود داشت. نتیجه‌گیری: عدم هم‌راستایی کشککی-رانی با نقص‌غضروف مفصل زانو همراه است. نتایج حاصل از‌این مطالعه برای پزشکان و متخصصان مفید خواهد‌بود و می‌توان با تشخیص زود هنگام از ایجاد آرتروز زانو جلوگیری کرد. در نهایت مطالعه حاضر ارتباط معناداری را بین پارامترهای مختلف هم‌ترازی مفصل زانو و نقص‌غضروف نشان‌ داد.   <div>Background: This cross-sectional study aims to evaluate the association between patellofemoral joint alignment measurements and patellofemoral cartilage defects using MRI images. Methods: A retrospective and cross-sectional study was conducted on 530 knee MRI images from Golestan and Imam Khomeini hospitals in Ahvaz, Khuzestan, Iran. The images were obtained from individuals experiencing non-traumatic knee pain and were selected based on their age, with subjects over 30 years of age being included in the study. This survey was conducted over the period from March 2023 to March 2024. The parameters of femoral groove angle (SA), femoral groove depth (SD), lateral patellofemoral angle (LPFA), tibial-trochlear tubercle groove distance (TT-TG), lateral patellar displacement (LPD), patellar inclination angle, and patellar height were evaluated. In addition, the Caton-Deschamps index and the degree of focal cartilage defect (from 0 to III) in the knee joint were evaluated.</div> <div> <table align="center" hspace="0" vspace="0"> <tbody> <tr> <td align="left" style="padding-top:0in; padding-right:12px; padding-bottom:0in; padding-left:12px" valign="top"> <div>Results: Regarding body mass index, 15.7% of the patients studied were underweight, 42.3% were of normal weight, 31.1% were overweight, 10.4% were obese, and 0.6% were extremely obese. There was a significant relationship between body mass index and patellar-lateral articular cartilage defects in the lateral tibia and lateral femur. There was a significant relationship between age and patellar-lateral articular cartilage defects in the lateral femur. There was a significant relationship between lateral patellar articular cartilage defects and lateral patellar dislocation, lateral patellofemoral angle, tibial prominence-skeletal groove distance, and patellar inclination angle. In addition, there was a significant relationship between medial patellar articular cartilage defects and knee groove angle. Conclusion: Patellofemoral misalignment is multivariately associated with knee articular cartilage defect. The results of this study will be of use to medical professionals, and the early diagnosis of knee osteoarthritis can prevent the development of the condition.
