fa بررسی 110 مورد هیپرپاراتیروئیدی اولیه در بیمارستان دکتر شریعتی و نتایج لوکالیزاسیون آدنوم توسط اسکن MIBI <p dir="rtl" align="right"><font face="tahoma,arial,helvetica,sans-serif">هیپرپاراتیروئیدی اولیه بیماری نسبتا شایعی است که در زنان به مراتب شایعتر از مردان می باشد. درمان آن جراحی و شامل برداشتن آدنوم پاراتیروئید یا غدد پاراتیروئید هیپرپلاستیک است. به علت مشکل بودن عمل جراحی از تستهای لوکالیزاسیون شامل سونوگرافی، CT scan ،MRI، اسکن تفریقی تالیوم-تکنسیوم و اسکن MIBI و یک سری تستهای دیگر کمک گرفته می شود که طی مطالعات انجام شده در میان این تستها اسکن MIBI بر سایرین ارجحیت دارد. ما در این بررسی 110 بیمار مبتلا به هیپرپاراتیروئیدی اولیه را که در فاصله سالهای 1376-1356 در بیمارستان دکتر شریعتی تحت عمل جراحی قرار گرفته اند مورد مطالعه قرار دادیم. جهت لوکالیزاسیون آدنوم در 10 نفر از بیماران سونوگرافی، در 38 نفر اسکن تالیوم تکنسیوم، در 30 نفر اسکن MIBI و در 3 بیمار CT scan انجام شده بود. در این بررسی حساسیت اسکن MIBI در حدود 86% و ویژگی آن 100% و حساسیت اسکن تفریقی تالیوم-تکنسیوم حدود 65% بدست آمد. این بررسی نشان می دهد که اسکن MIBI در بیماران ما از حساسیت و ویژگی بالایی برخوردار بوده و بهترین تست لوکالیزاسیون می باشد، بطوری که انجام آن را در تمامی این نوع بیماران توصیه می کنیم. </font></p> Primary Hyperparathyroidism is a fairly common disease that is much more predominant in females. Treatment is surgery and includes removing the parathyroid adenoma or the hyperplastic parathyroid glands. Considering the difficulty of this operation procedure, localization test are utilized including ultrasound, MRI, CT scan, Thallium-Technesium subtraction scan, MIBI scan and a series of other tests. According to the studies regarding these tests, MIBI scan is superior to all the others. In this survery we have studied 110 primary hyperparathyroidism patients that were operated on in Dr.Shariati Hospital between 1356 and 1376. In order to localize the adenoma ultrasound was used in ten patients, thallium-technesium subtraction scan in 38 patients. MIBI scan in 30 patients and CT scan in 3 patients. The sensitivity of MIBI scan was 86% and its specificity was 100%. In comparison, the sensitivity of thallium-technesium scan was found to be about 65%. Our results show that MIBI scan has had a high degree of sensitivity and specificity in our patients and has shown to be the best localization test.
