fa عملکرد ریوی در اسکولیوز ایدیوپاتیک نوجوانان قبل از جراحی ستون فقرات و تاثیر نوع جراحی بر آن <p style="text-align: justify" dir="rtl"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--><!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--><strong>زمینه و هدف: </strong>اسکولیوز یکی از شایع‌ترین دفورمیتی‌های ستون فقرات می‌باشد و موجب کاهش عملکرد ریوی می‌شود. در مورد تغییرات عملکرد ریوی پس از اصلاح جراحی در اسکولیوز ایدیوپاتیک نوجوانان اختلاف نظر وجود دارد. هدف از این مطالعه بررسی تغییرات ریوی پس از جراحی با توجه به نوع جراحی انجام شده در اسکولیوز ایدیوپاتیک نوجوانان بود.</p> <p style="text-align: justify" dir="rtl"><strong>روش بررسی: </strong>65 بیمار با تشخیص اسکولیوز ایدیوپاتیک نوجوانان که در بیمارستان شفا یحیاییان از سال 1376 تا 1386 تحت عمل جراحی اصلاحی ستون فقرات قرار گرفته بودند و اطلاعات مربوط به آزمون عملکرد ریوی قبل از انجام جراحی آن‌ها موجود بود، وارد مطالعه شدند. بیماران بر اساس نوع جراحی به سه گروه تقسیم شدند. گروه اول شامل 25 بیمار با فیوژن خلفی (PSF)، گروه دوم شامل 29 بیمار با فیوژن قدامی و خلفی توام (ASF+PSF) و گروه سوم شامل 10 بیمار با فیوژن خلفی به‌علاوه توراکوپلاستی (PSF+Thoracoplasty) بود. مقادیر مطلق و درصد پیش‌بینی شده ظرفیت حیاتی حداکثر (FVC) و حجم بازدمی ثانیه اول (FEV 1) قبل از جراحی و در آخرین پی‌گیری بیمار با هم مقایسه شدند و رابطه آن‌ها با نوع جراحی، بررسی شد.<strong> </strong><strong></strong></p> <p style="text-align: justify" dir="rtl"><strong>یافته‌ها:</strong> هیچ رابطه معنی‌داری بین نوع جراحی انجام شده و میزان تغییرات آزمون عملکردی ریوی وجود نداشت (05/0P>). میزان FVC و FEV1 بیماران پس از سپری شدن حداقل دو سال از زمان جراحی در گروه‌های بیماران همانند مقادیر FVC و FEV1 قبل از جراحی بود.<strong> </strong></p> <p style="text-align: justify" dir="rtl"><strong>نتیجه‌گیری: </strong>پس از گذشت حداقل دو سال از زمان جراحی بین روش‌های جراحی از نظر نتایج آزمون عملکرد ریوی تفاوتی موجود نبود.</p> <p style="text-align: justify"><strong>Background: </strong>Scoliosis is one of the most common spinal deformities with subsequent decrease in pulmonary function. The effects of surgical correction on the pulmonary function of patients with adolescent idiopathic scoliosis are controversial. The purpose of the present study was to compare the postoperative pulmonary function changes in different surgical approaches chosen for its correction.<br><strong></strong></p> <p style="text-align: justify"><strong>Methods: </strong>Sixty-five patients with adolescent idiopathic scoliosis who had undergone corrective spinal surgery in Shafa Yahyaian Hospital since 1997 to 2007 and had documented preoperative pulmonary function test report, included in our study. The patients had documented preoperative pulmonary function tests and were divided into three groups based on their surgical approach. The first group was comprised of 25 patients who had undergone posterior spinal fusion (PSF), the second group included 29 patients who had anterior and posterior spinal fusion (ASF+PSF), and the third group consisted of 10 patients who had posterior spinal fusion and thoracoplasty (PSF+ thoracoplasty). The preoperative and the final follow-up values of absolute and percent predicted forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were compared and their relations with the undertaken surgical approach were evaluated.<br><strong></strong></p> <p style="text-align: justify"><strong>Results:</strong> There were no significant correlations between the surgical approaches and changes in the pulmonary function tests (P>0.05).
