fa شناسایی عفونت ویروس اپشتین- بار در بیماران مبتلا به لنفوم: آزمون ELISA و PCR <p dir="rtl" style="text-align: justify"><!--stripped--> 800x600 <!--stripped--> <strong>زمینه و هدف: </strong>ژنوم فاز نهفته عفونت ویروس اپشتین- بار (EBV) در سلول‌های بدخیم تقریباً در یک‌سوم موارد بیماری لنفوم هوچکین یافت می‌شود. شناسایی DNA ویروسی می‌تواند به‌طور بالقوه به‌عنوان یک بیومارکر از فعالیت بیماری به‌کار رود. <strong><br></strong></p><p dir="rtl" style="text-align: justify"><strong>روش بررسی: </strong>از نمونه خون 44 بیمار مبتلا به لنفوم و 44 فرد کنترل جهت استخراج DNA و اندازه‌گیری تیتر IgG ضد EBNA-1 ویروس استفاده شد. همزمان بلوک‌های پارافینه هر فرد بیمار نیز مورد استخراج DNA قرار گرفت. جهت شناسایی ویروس آزمون Nested PCR که هر دو تیپ ویروس را شناسایی می‌کردند، انتخاب شدند.</p><p dir="rtl" style="text-align: justify"><strong>یافته‌ها:</strong> از 44 بیمار به‌ترتیب (1/84%)37 آزمون ELISA، (3/27%)12 Blood PCR و (6/13%)6 Biopsy PCR و از افراد کنترل نیز به‌ترتیب (7/47%)21 و (16%)7 آزمون ELISA و Blood PCR مثبت داشتند. نتایج آزمون‌های ELISA و Blood PCR و Biopsy PCR مثبت در گروه بیماران هوچکینی و غیرهوچکینی به‌ترتیب (84%)21، (24%)6، (16%)4 و (2/84%)16، (6/31%)6، (5/10%)2 بوده است. مقایسه نتایج این آزمون‌ها بین دو گروه بیماران تفاوت معنی‌داری را نشان نداد (مقادیر p برای ELISA، Blood PCR و Biopsy PCR به‌ترتیب 26/0، 73/0 و 68/0 بوده است). <br></p><p dir="rtl" style="text-align: justify"><strong>نتیجه‌گیری: </strong>نتایج مقایسه ELISA و Blood PCR این مطالعه در طیف سنی اطفال و بالغین بیمار با طیف سنی مشابه در گروه کنترل نشان داد که تنها در آزمون ELISA و فقط در بین اطفال تفاوت معنی‌دار وجود داشت و نتایج حاصل از PCR معنی‌دار نبود. به‌علاوه نتایج آزمون‌ها بین بیماران هوچکینی با گروه کنترل فقط در نتایج ELISA دارای تفاوت معنی‌دار بود. نتایج آزمون‌های بیماران هوچکینی و غیر هوچکینی در هیچ‌یک از آزمون‌ها تفاوت معنی‌دار نشان نداد.</p> <p><!--stripped--> Normal 0 false false false EN-GB X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--><!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:10.0pt font-family:"Times New Roman","serif"} </style> <!--stripped--></p> <p style="text-align: justify"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0in 5.4pt 0in 5.4pt mso-para-margin:0in mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>Background:</strong><strong> </strong>Latent Epstein- Barr virus (EBV) genomes are found in the malignant cells of approximately one-third of Hodgkin's lymphoma (HL) cases. Detection of EBV viral DNA could potentially be used as a biomarker of disease activity. Our goal was to compare of EBV DNA detection in samples obtained from lymphoma patients versus controls.<br><strong>Methods:</strong><strong> </strong>One milliliter uncoagulated and 1ml coagulated blood sample for DNA extraction and serum analysis using ELISA for IgG anti EBNA-1 were obtained from 44 lymphoma patients and from 44 normal controls, respectively. EBV genome, EBNA-2, was examined from DNA extracts of paraffin embedded and blood samples using Nested PCR with type specific inner primers.<br><strong>Results:</strong><strong> </strong>Positive results for ELISA, Blood and biopsy PCR in study group were, 84.1%, 27.3% and 13.6%, respectively. However, these results in control group were 47.7% and 16% for ELISA and Blood PCR assays, respectively. Positive results in ELISA, Blood PCR and Biopsy PCR in Hodgkin and non-Hodgkin patients were found in 21(84%), 6(24%), 4(16%) and 16(84.2%), 6(31.6%), 2(10.5%) of specimens, respectively. No significant differences in EBV detection were found between these two patient groups (p values for ELISA, Blood PCR and Biopsy PCR were 0.26, 0.73 and 0.68, respectively).<br><strong>Conclusion: </strong>Comparison of ELISA and Blood PCR results in children and adult patients with the same age of controls have showed difference in ELISA results of children, only. None of the test results have showed statistically significant difference between Hodgkin and non-Hodgkin patients. However, the mean of ELISA results in Hodgkin patients was higher as compared with controls.
