fa بررسی کلینیکوپاتولوژیک موارد لنفوم سلول بزرگ: بررسی 1470 نمونه بیوپسی <p style="text-align: justify" dir="rtl"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0in 5.4pt 0in 5.4pt mso-para-margin:0in mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>زمینه و هدف</strong>: لنفوم‌ها گروه شایعی از نئوپلاسم‌ها را تشکیل می‌دهند. هدف از این مطالعه گذشته‌نگر بررسی کلینیکوپاتولوژیک موارد لنفوم غیر هوچکین با سلول بزرگ (Diffuse large cell) و ارتباط طبقه‌بندی پاتولوژیک با مشخصات بالینی و سیر بیماری است. <strong></strong></p> <p style="text-align: justify" dir="rtl"><strong>روش بررسی</strong>: اطلاعات مربوط به تشخیص و درمان کلیه بیماران با تشخیص پاتولوژی لنفوم غیر هوچکین مربوط به دوره 22 ساله از سال 1364 لغایت 1385 در مرکز سانترال بیمارستان امام‌خمینی جمع‌آوری شد. بلوک بیوپسی 50 بیمار مبتلا که اطلاعات کامل از سیر بالینی بیماری ایشان موجود بود انتخاب شده و مورد بازبینی آسیب‌شناسی و بررسی ایمونوهیستوشیمی قرار گرفت. زیر گروه بیماری در طبقه‌بندی Stage, WHO، مشخصات ایمونوهیستوشیمی و سیر بیماری بررسی شد. <strong></strong></p> <p style="text-align: justify" dir="rtl"><strong>یافته‌ها</strong>: 220 مورد لنفوم large cell شناسایی گردید. 55 مورد بلوک پاتولوژی در انستیتو کانسر موجود بود که مورد بازبینی و بررسی ایمونوهیستوشیمی قرار گرفت. از 55 گزارش large cell پنج مورد لنفوم T Cell مشخص گردید. در مورد 50 بیمار باقیمانده میانه سنی 45 سال و 60% بیماران مرد بودند. 24% سمپتوم B و 28% شروع بیماری خارج از غدد لنفاوی داشتند. تمام بیماران شیمی درمانی و 46% رادیوتراپی شدند. 20% عود بیماری و 13 مورد مرگ ذکر گردید. میانه طول عمر بیماران 34 ماه و طول عمر پنج ساله 45% بود. <strong></strong></p> <p style="text-align: justify" dir="rtl"><strong>نتیجه‌گیری</strong>: مشخصات بالینی و پاتولوژی بیماران با آمارهای بین‌المللی مطابقت داشته است و استفاده از ایمونوهیستوشیمی کمک ارزنده‌ای به تشخیص موارد لنفوم سلول B بزرگ نموده است.</p> <p style="text-align: justify"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0in 5.4pt 0in 5.4pt mso-para-margin:0in mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>Background</strong><strong>:</strong> Diffuse large B Cell lymphoma (DLBCL) is the most common subtype of non-Hogkin lymphoma (NHL). We performed a retrospective study of patients with de novo DLBCL treated in the Medical Oncology department of Cancer Institute of Iran, Tehran to assess the clinicopathologic and immunohistochemistry correlation and prognosis of the patients. <br><strong>Methods</strong><strong>:</strong> World Health Organization (WHO) classification was used to reexamine 1470 biopsy specimens related to the years 1985-2006. After excluding five cases of T Cell large cell lymphoma, 50 Patients diagnosed as DLBCL. <br><strong>Results</strong><strong>:</strong> Median age of the patients was 45.5(20-85) years: 60% were male and 30% had primary extranodal disease. The most common extranodal sites were bone, gastrointestinal tract and Head and neck areas. The most common stages were stage II (32%), stage III (32%), stage IV (20%) and stage I (16%) retrospectively and 33% had B-symptoms. All of The Patients received chemotherapy (83% CHOP regimen) and 46% treated by radiotherapy after chemotherapy. With a mean follow up time of 32 months, median survival time was 34 (95% CI 24-40) months. Prognostic factors for survival were tumor stage, B-symptoms and early relapse (less than 6 months).<br><strong>Conclusions:</strong> Our data showed the importance of Immunohistochemistry method in diagnosis of DLBCL.
