fa تشخیص سریع کمپلکس مایکوباکتریوم توبرکلوزیس: روش PCR با استفاده از قطعه الحاقی 6110 <p dir="rtl" style="text-align: justify"><!--stripped--> Normal 0 false false false EN-GB X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>زمینه و هدف: </strong>سل هنوز یکی از مهم‌ترین علل مرگ و میر در بسیاری از کشورهاست. با توجه به وقت‌گیر بودن روش‌های معمول تشخیص سل مانند روش کشت، لازم است روش‌های سریع تشخیصی نظیر PCR مورد ارزیابی قرار گیرد. با توجه به اهمیت درمان سریع سل و به‌منظور تعیین حساسیت و ویژگی روش سریع تشخیص مایکوباکتریوم توبرکلوزیس با استفاده از قطعه الحاقی 6110 (IS6110) به‌روش PCR این مطالعه در دانشگاه علوم پزشکی کاشان انجام پذیرفت. <br></p><p dir="rtl" style="text-align: justify"><strong>روش بررسی: </strong>این مطالعه از نوع ارزش تشخیصی بوده که بر روی 248 نمونه خلط بیماران مشکوک به سل مراجعه‌کننده به درمانگاه مسلم کاشان در 1386 انجام پذیرفت. نمونه خلط پس از انجام هموژنیزاسیون و آلودگی‌زدایی بر روی محیط لون اشتاین جانسون کشت گردید، DNA مایکوباکتریوم توبرکلوزیس، با استفاده از روش جوشاندن استخراج گردید. سپس دو قطعه 123 جفت بازی و 245 جفت بازی از قطعه الحاقی 6110 تکثیر و با استفاده از الکتروفورز ژل آگارز شناسایی گردید. حساسیت، ویژگی، ارزش تشخیصی مثبت و منفی روش سریع تشخیص مایکوباکتریوم توبرکلوزیس با استفاده از قطعه الحاقی 6110 تعیین گردید.<strong><br></strong></p><p dir="rtl" style="text-align: justify"><strong> </strong><strong>یافته‌ها:</strong> در این مطالعه از مجموعه 248 نمونه خلط اخذ شده از 124 بیمار 40 نمونه اسمیر مثبت (1/16%) و 32 نمونه دارای نتیجه کشت مثبت (9/12%) بودند. حساسیت، ویژگی روش سریع تشخیص مایکوباکتریوم توبرکلوزیس با استفاده از قطعه الحاقی 6110 به‌ترتیب 75/93 و 1/99 درصد بود.</p><p dir="rtl" style="text-align: justify"><strong> نتیجه‌گیری: </strong>روش سریع تشخیص مایکوباکتریوم توبرکلوزیس با استفاده از قطعه الحاقی 6110 نسبت به کشت دارای سرعت زیاد، حساسیت مناسب و ویژگی بالایی می‌باشد.</p> <p style="text-align: justify"><!--stripped--> Normal 0 false false false EN-GB X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>Background:</strong><strong> </strong>Tuberculosis is an important cause of death in some countries. The world health organization estimates that if stronger measures are not taken up to control the prevalence of this disease, from 2000 to 2020 a billion people will be infected by the bacterium. According to time consuming of common detection methods of Mycobact-erium tuberculosis such as culture, it is necessary to evaluate a rapid detection tests such as PCR. Rapid diagnosis of tuberculosis may have profound effects in patients' care According to importance of rapid detection and treatment of tuberculosis and for determine of sensitivity, specificity, positive predictive value and negative predictive value of PCR by using IS6110 this study was done in Kashan university of medical science.<br><strong>Methods:</strong><strong> </strong>A total of 248 sputum samples from patients suspected of mycobacterial diseases were studied. DNA was extracted by boiling method. IS6110 PCR method by a specific pair of primers designed to amplify 123bp and 245bp sequences of the insertion sequence, 6110, in the M. tuberculosis genome was used to analyze sputum samples.<br><strong>Results:</strong><strong> </strong>32 out of 248 (12.9%) of samples had positive culture. PCR yielded a sensitivity of 93.8<strong>%</strong> and specificity of 99.1% for the diagnosis of TB patients with TB confirmed by culture.
