fa ارایه مدلی برای بهبود پرونده‌های پزشکی با ایجاد پرونده الکترونیک سلامت: مقاله مروری مقاله مروري Review Article <div style="text-align: justify;"><span style="font-family:yekanyw;"><span style="font-size: 10pt;">امروزه کاربردهای فناوری</span> <span style="font-size: 10pt;">داده‌ها و ارتباطات</span> <span style="font-size: 10pt;">به‌عنوان عاملی مهم و کلیدی در پیشرفت همه سازمان‌ها از جمله سازمان‌های نظام سلامت و بهداشت محور می‌باشد. با عنایت به اهمیت موضوع فوق، این سازمان‌ها در جهت متعالی‌سازی سیستم‌های در حال استفاده خود، دگرگونی و تغییر و تحول خاصی ایجاد نمودند که</span> <span style="font-size: 10pt;">یکی</span> <span style="font-size: 10pt;">از</span> <span style="font-size: 10pt;">این تغییرات ایجاد پرونده‌های الکترونیک سلامت </span><span dir="LTR"><span style="font-size: 8pt;">(Electronic Health Records, EHR)</span></span><span style="font-size: 10pt;"> می‌باشد. این سیستم متحول‌کننده با ایجاد بهره‌وری هم از حیث افزایش کارایی پرسنل و هم از حیث افزایش اثربخشی در فرآیند درمان، مسیر تشخیص تا درمان را ساده‌تر نموده و عملا از ارایه گزارش‌های مکتوب و حجیم درمانی ممانعت به‌عمل می‌آورد. با عنایت به رعایت اصول اخلاقی در حفظ اسرار بیماران و محرمانه بودن اطلاعات آن‌ها، نحوه‌ی بایگانی نمودن اسناد پزشکی الکترونیک در پایگاه داده‌های امن، از اهمیت بالایی برخوردار است. این مهم از مهمترین مباحث اصول اخلاقی بوده و مدیران بیمارستانی می‌بایست سازوکاری تهیه نمایند تا داده‌های تمامی بیماران به‌طور مناسب ذخیره و نگهداری گردد. در این مقاله سعی شده مدلی برای نسخه بیمارستانی پرونده الکترونیک سلامت ارایه شود که بسیاری از بیمارستان‌ها بتوانند از این مدل برای بهینه‌سازی سیستم‌های ثبت پرونده پزشکی خود استفاده کنند. هدف از ارایه این مدل، تسریع و به‌کارگیری فرآیند ایجاد پرونده‌های الکترونیکی در نظام سلامت، به‌ویژه بیمارستان‌های دولتی کشور با حجم وسیعی از بیماران، می‌باشد. با به‌کارگیری این مدل، تعامل درون سازمانی و برون سازمانی برای سازمان تسهیل گردیده و امکان پاسخگویی چابک در زمان معین فراهم می‌گردد. علاوه بر موارد فوق با به‌کارگیری و عملیاتی شدن مدل، امکان کاهش حجم انتقادات و شکایات از بیمارستان‌ها نیز تحقق خواهد یافت و میزان استفاده از داروها بر اساس نیاز واقعی جامعه و مبتنی بر داده‌های فردی صورت خواهد پذیرفت. اجرای این مدل موانعی نیز در بر دارد که در مقاله به نمونه‌هایی از موانع اجرایی اشاره شده است ولی به‌منظور غلبه بر این موانع، نیازمند تلاش مدیریتی بیشتر و حمایت‌های گسترده‌تر دولتی می‌باشد. این تلاش‌ها در بستر فرهنگ‌سازی استفاده از فناوری اطلاعات چه در میان پرسنل بیمارستانی و چه در میان بیماران و مراجعان نظام سلامت از اهمیت بالایی برخوردار است.</span></span><br> &nbsp;</div> <div data-select-link-text="1" style="text-align: justify;">Today, the use of information and communication technology (ICT) is an important and key factor in the progress of all organizations, including health-centered and health systems. Given the importance of the subject matter above, these organizations have created a particular transformation and change in order to upgrade their systems in use, one of which is the creation of Electronic Health Records (EHR). This evolving system, by increasing productivity, both by increasing staffing efficiency and by increasing the effectiveness of the treatment process, simplifies the diagnosis path to treatment and prevents the submission of written and bulky reports. Given the ethical principles of protecting the privacy of patients and the confidentiality of their information, how to archive electronic medical records in a secure database is very important. This is one of the most important issues of ethics and hospital managers should provide mechanisms to keep all patient data properly stored and maintained. In this paper, we have tried to provide a model for the Electronic Health Record Hospital, which many of them could use to optimize their medical records systems. The purpose of this model is to accelerate and apply the process of creating electronic records in the health system, especially government hospitals with a large number of patients. By using this model, internal and external interaction of organization is facilitated, and agile responsiveness can be provided at a given time. In addition to the above, with the implementation and operationalization of the model, the possibility of reducing the volume of criticisms and complaints from hospitals will also be realized and the use of drugs will be based on the actual needs of the community and based on individual data. Implementation of this model also has barriers to addressing some of them in this article, but in order to overcome these barriers, more administrative effort and wider governmental support are needed.
